== Cause == Mutations in the UGT1A1 gene lead to Gilbert syndrome. The gene provides instructions for making the bilirubin uridine diphosphate glucuronosyltransferase (bilirubin-UGT) enzyme, which can be found in the liver cells and is responsible for preparing bilirubin for removal from the body. The bilirubin-UGT enzyme performs a chemical reaction called glucuronidation. Glucuronic acid is transferred to unconjugated bilirubin, which is a yellowish pigment made when your body breaks down old red blood cells, and then being converted to conjugated bilirubin during the reaction. Conjugated bilirubin passes from the liver into the intestines with bile. It's then excreted in stool. People with Gilbert syndrome have approximately 30% of normal bilirubin-UGT enzyme function, which contributes to a lower rate of glucuronidation of unconjugated bilirubin. This substance then accumulates in the body, causing mild hyperbilirubinemia.

== Biography == Meldal received B.S. and PhD degrees in chemical engineering from Technical University of Denmark (DTU); his PhD work was supervised by Klaus Bock and focused on the synthetic chemistry of carbohydrates. From 1983 to 1988 he was a postdoctoral fellow in organic chemistry, first at the DTU, next at the MRC Laboratory of Molecular Biology at Cambridge University and then at the University of Copenhagen. In 1996 he was appointed assistant professor at DTU. Since 1998 he has led the synthesis group in the Department of Chemistry of the Carlsberg Laboratory. Meldal developed several technological techniques and instruments for peptide synthesis near the start of his career. == Treatment == Symptomatic individuals should be seen by an orthopedist to assess the possibility of treatment (physiotherapy for muscular strengthening, cautious use of analgesic medications such as nonsteroidal anti-inflammatory drugs). Although there is no cure, surgery is sometimes used to relieve symptoms. Surgery may be necessary to treat misalignment of the hip (osteotomy of the pelvis or the collum femoris) and, in some cases, malformation (e.g., genu varum or genu valgum). In some cases, total hip replacement may be necessary. However, surgery is not always necessary or appropriate. Sports involving joint overload are to be avoided, while swimming or cycling are strongly suggested. Cycling has to be avoided in people having ligamentous laxity. Weight control is suggested. The use of crutches, other deambulatory aids or wheelchair is useful to prevent hip pain. Pain in the hand while writing can be avoided using a pen with wide grip.

In one family, the only notable finding on T1-weighted MR images (used to detect fatty infiltration) was atrophy of the rectus femoris muscles of the thigh, with the degree of atrophy matching the severity of the disease, but no fatty infiltration. In another family, only the more severely affected older patient showed significant abnormality, by having symmetrical fatty atrophy of the femoral quadriceps of the thigh, the adductor and medial gastrocnemius muscles of the calf; as well as asymmetrical fatty atrophy of the adductor longus of the thigh. No muscle hypertrophy was reported and the muscles of the patients without fatty atrophy appeared normal. Bethlem myopathy 2 also differs by including the possibility of scapula winging, pectus excavatum, stooped posture, kyphosis (hunchback), micrognathia, retrognathia, and a high-arched palate. == Chemistry and pharmacokinetics == Phenformin hydrochloride is a white crystalline powder, with a melting point of 175–178 °C; it is soluble at 1 in 8 parts of water and 1 in 15 of ethanol, and practically insoluble in chloroform and ether. Phenformin is less polar and more lipid soluble and exhibits a higher affinity for mitochondrial membranes than metformin. Its dissociation constant (pKa) is 2.7, 11.8 (at 32 °C), and log P = –0.8. Phenformin is well absorbed after oral administration. The major metabolic reaction is aromatic hydroxylation to form 4-hydroxyphenformin, which is then conjugated with glucuronic acid. Up to about 50% of a dose is excreted in the urine in 24 h, about two–thirds in the form of unchanged drug and one–third as the hydroxy metabolite. Following a single oral dose of 50 mg to eight subjects, peak plasma concentrations of 0.08–0.18 mg/L (mean 0.13) were attained in about 3 h; plasma concentrations were higher in four subjects who were poor metabolisers of debrisoquine in comparison with the four extensive metabolisers. === North Korean involvement === In August 2018, Reuters reported that a confidential United Nations investigation had found the North Korean government had failed to discontinue its nuclear and missile delivery programs, and in conjunction, was "cooperating militarily with Syria" and was "trying to sell weapons to Yemen's Houthis". In August 2019, the South Korean National Intelligence Service had tracked the Scuds missiles (used to attack Saudi Arabia) back to North Korea. In January 2024, South Korea's Yonhap News Agency reported that North Korea had evidently shipped weapons to Houthis via Iran, based on the writings in Hangul script that were found on missiles launched towards Israel. == History == Waterlogged wood is a wooden object that has been submerged or partially submerged in water and has affected the original intended purpose or look of the object. Waterlogged wood objects can also include wood found within moist soil from archaeological sites, underwater archaeology, maritime debris, or damaged wood objects. Conservation of waterlogged wood has changed throughout time. Due to the delicate structure of this organic material, conservators and museum professionals alike have struggled with finding a textbook method. An example of the conservation of wood and how it has changed over time can be illustrated by the conservation treatments of a Kefermarkt altar. The earliest attempt at wood conservation is recorded as late as 1852–1855, A. Stifter treated the Kefermarkt altar in Austria with table salt in an attempt to protect the wood against pests. In around 1916-1918 Councillor Bolle attempted to protect the wood altar from pests included brushing the wood with petroleum and hexachloroethane. Finally, in 1929 the use of hydrogen cyanide relieved the pest problem and preserve the woods longevity. From a conservation standpoint, waterlogged or not, wood has been difficult to preserve because it is an organic material.
